How patients consent to texts
Phoenix Wellness
Phoenix Wellness collects consent in person at the clinic. There is no advertisement, keyword, website form, or QR code for first-time opt-in. Agreeing to texts is optional. A patient who says no, leaves the SMS box unchecked, or whose record is marked do-not-text is never messaged and still receives care.
Verbal script
Staff read the following aloud and record the yes-or-no on the patient record before any text is sent.

Paper intake form
This is the intake form used at the clinic. Name, phone, and signature are for care. The SMS box is a separate, unchecked, optional question. A patient can complete every other field and receive care without agreeing to texts.
